Below are the various forms needed for the AHS Orchestra, including fundraising forms, permission forms, instrument usage forms, etc. Click on each form name to download a pdf copy. Read the descriptions below each for information about due dates and submission instructions.
Instrument Usage FormFor use primarily by cello and bass players, this form is necessary when checking out an AHS-owned instrument. Any violinists or violists who will need to use a school instrument should also use this form.

AHS Orchestra ContractThe orchestra contract is an agreement, submitted at the end/beginning of every year, between the parents, students, and orchestra director. It states that all will do their part to make orchestra a positive experience and also grants permission for field trips and student photos on the website. Returning students will submit in April each year. New incoming freshman will submit their forms no later than the second week of August.

T-shirt Order FormEvery year, the AHS Orchestra creates a t-shirt that can be worn by the students to show their pride at being a member of the ensembles. Click here for the form to purchase a shirt.
